Documents Required

• Original CNIC/B-Form or original CNIC and photocopy
• Original passport and a copy
• Affidavit
• Authorization Letter (If the applicant is located abroad). An authority letter stamped with the relevant Embassy stamp will be accepted. Otherwise, it should be attested in writing by two persons known to the applicant. The authority letter should mention the duration of the applicant’s stay.
• Photograph (If a certificate of blood relatives is required)
• The form can be completed by any relative of the applicant (brother or father, mother, sister, etc.) if the applicant is not in the country. With an authority letter from the applicant.

Processing Fee

• Rs. 350. You can deposit the fee at any of the PKM counters
